Winter 2020-2021 100th Season Jersey Order Information:
$179.99+tax for a blank Jersey (Chest Logo, 100th season patch, MCM shoulder patches) paid to University Images separately from this form. Embellished jerseys are an additional $75 which includes a nameplate and or player numbers. A captain’s letter is an additional $5.
Timing of this jersey order is to be determined but blank jerseys could arrive before the 2021-22 season starts and embellished jerseys are usually available around Christmas time.
The current deadline to complete this form (and order from University Images) and send payment is March 15, 2021.
